Please download these forms. Forms are fillable PDF’s or need to be printed for appropriate signatures. Forms can be completed, saved and/or scanned with student last name and first initial (i.e. RasmussenN) and emailed to jhernandez@cybervillageacademy.org or mailed to CVA 3810 E 56th St. Minneapolis, MN 55417, Attn. Jennifer. 

• Educational Benefits Form (All Students – School funding and for determining eligibility for free or reduced cost lunches, P-EBT, etc.) (Required)
• MDE Immunization Records (Students entering Grades 7 or 12 – Required): Physician office completes or printed immunization record
• Medication Authorization Form (Optional – if needed)
